Hudson Headwaters Health Network (HHHN) is a nonprofit system of 23 community health centers providing primary care to New York’s Adirondack North Country and Glens Falls region. HHHN is the only medical provider in much of its region, so having a helpful and easy-to-use website is critical. After years of temporary fixes, the site needed a complete overhaul.

Fulfilling a long wish list with high stakes
With more than 20 service lines and 220+ providers in network, Hudson Headwaters Health Network’s website needed to support a huge volume of information. We also had to address the constant burden of managing all that content—a real pain point for HHHN’s internal teams. Finally, we had to consider both their unique mobile health service and brick-and-mortar locations to provide consistent, frictionless user experiences for each.
We kicked off the project with a thorough planning phase spanning user research, journey mapping, a content audit, requirements planning, and technology assessment.

Connecting patients to care faster and more easily
Flynn developed an optimized site map and multiple visual approaches to provide HHHN with options for the site blueprint. Page designs and layouts followed, along with a plan to integrate several existing third-party applications into the site to support tasks ranging from scheduling appointments to fundraising.
Among other patient-friendly features, the new site’s reimagined homepage helps visitors find key content faster. Users can also search for in-network providers by location, services and specialties, and whether they’re accepting new patients. All helping improve access to care and patient-provider fit while increasing scheduled appointments.
